Our team are registered medical and allied health professionals who are passionate about helping you to ensure that your weight management requirements are adequately met and that you receive the most appropriate education and support regarding all elements of your weight loss journey.
We understand that weight management is a complex process that involves a range of factors and for many people simple exercise and nutrition is not enough.
We know the impact that weight problems can have on an individual’s health, confidence, relationships and general enjoyment of life – and the great benefits that even small weight losses can bring.
Our team consists of Nurse Practitioners, Nursing team, Visiting Medical Specialists, Credentialled Diabetes Educators, Dietitians, Physiotherapists, Psychologists, Exercise Physiologists & Practice support team.
Our Nurse Practitioners are qualified to medically assess, order diagnostic tests, diagnose, devise a treatment plan for a patient and prescribe autonomously within their outlined scope of practice.
Our Transformed Nurse Practitioners are also trained and certified in weight loss through the Australian College of Weight Management.
The Nurse Coordinator will help you work out what services we will provide you with to ensure you achieve your goal. Please remember the Care plan that you MAY BE entitled to from your GP only entitles you to a total of 5 ALLIED HEALTH visits per year.
These will not be enough to fully service your needs, so some visits will need to be self- funded – if you have Private Health insurance you can also Claim these visits here.
Nursing - Weight Loss Coaches
The Transformed program uses a combination approach to weight loss which includes a series of eating plans, weekly one-on-one coaching sessions and very specific personal care guidelines, to support effective, attainable weight loss.
Regular contact with your trained Transformed Coach and weekly face to face consultations meetings provide the framework necessary for this program to be administered safely and correctly with maximum weight loss results and long term success. The real key to the success of the transformed program is the quality of the coaching, advice and support given to each client throughout their weight loss journey.
Our Transformed Coaches are ready to assist you to achieve your weight loss goals and then offer you the tools to maintain your new weight long-term.
Visiting Medical Specialist
Credentialled Diabetes Educator
Our Diabetes Educators are able to assist people to live well with diabetes. They can assist with insulin therapy, blood glucose monitoring and guidance with food choices and mindful eating practices for maintaining a healthy weight.
Accredited Practising Dietitians (APD) are credentialled Dietary specialists who have studied the science of human and medical nutrition.
They can help people understand the relationship between food and health so they can make personalised dietary choices to attain and maintain health, whilst preventing and treating illness and disease.
Our APD's will provide you with simple, sustainable and individualized nutrition solutions that fit with your short-term and long-term goals in line with any existing issues and nutritional requirements.
Physiotherapists are university qualified allied health professionals who design, deliver and evaluate safe and effective exercise interventions for people with acute, sub- acute or chronic medical conditions, injuries or disabilities.
They will prescribe exercise plans to improve your levels of fitness for general health and to increase and sustain your weight loss outcomes including clinical pilates and home based exercise interventions.
Our Clinical Psychologist specialise in in Weight Management and Food Addiction.
They will assist you to look at any unhelpful thinking and behaviours related to food and dieting and develop strategies to help break these habits. By changing your thinking and behaviour, you are more likely to lose weight and maintain your goals for the long term.
